General Forms for 2007–2008
Parental Response Form	Explanation of child's strengths and weaknesses meant to facilitate overall growth throughout the academic year.
Parental Permission Form	Indicates your preferences for your child throughout the academic year regarding use of cars and public transportation, field trips, overnights, etc.
Athletics/Physical Education Form	Requests information about your child's participation in interscholastic sports, intramurals, and physical education classes.
Host Family Program Form	Allows you to request a host family for your child or become a host family for other students.
Instrumental Lessons Form	Students can be signed up for instrumental lessons. Due
Boarding Student Allowance Form	For parents who want to utilize the Business Office's allowance program for their student.
Bus Transportation Form	Sign up for bus transportation.
Phone Service Form	Sign up for long distance service in the dormitories.
Acceptable Use Policy	Agree to terms of the Acdemy's Student Acceptable Use Policy.
Outdoor Program Form	Indicates your preferences for your child throughout the academic year regarding outdoor program trips.
Emergency Contact Form	Provide contact information in the case of an emergency.

Health Center Forms 2008–2009
Letter from the Health Center	This letter from the Health Center explains the health-related forms.
Upper School and Middle School Medical Permission Form Lower School Medical Permission Form	Form that gives consent for the medical staff or their appointee to treat students. Due July 15.
Physical Exam Form Physician Information	Physician's report that the student received a complete physical examination. Due July 15.
Medical History Form	Listing of medical history and immunizations. To be completed by parent or guardian. Due July 15.
